We are committed to protecting the privacy of all visitors to our website, and we want you to understand how we protect, collect, and use your information. Please note that the following policy applies only to your use of our website. Our Health Insurance Portability and Accountability Act (HIPAA) notice of privacy practices, which governs your personal, protected health information (PHI) after clinical service engagement, can be found here

This website privacy policy applies to individually identifiable information that you provide to us. This privacy statement supplements the HIPAA notice for PHI. If there is ever any conflict between this privacy policy and the HIPAA notice, the HIPAA notice will apply. The HIPAA notice does not apply to information that is not PHI.

We collect and briefly store information that you provide directly to us. This includes information you submit on forms, such as contact request forms or donation forms. We only collect the minimum necessary information for us to be able to provide services to you as requested. We may collect some or all of the following information:​

We use reasonable security measures to protect the confidentiality of personal information under our control and appropriately limit access to it. We use a variety of information security measures to protect your online transactions with us. Our site uses encryption technology, including TLS 1.3 encryption and level 1 PCI compliance. All direct payment gateways used by our company adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express and Discover. PCI-DSS requirements help ensure the secure handling of credit card information by our donation form.

APWI cannot ensure or warrant the security of any information you transmit to us, and you do so at your own risk. We take reasonable steps to ensure the integrity and confidentiality of personally identifiable information that you may provide. However, electronic transmissions via the Internet are not necessarily secure from interception, and so we cannot absolutely guarantee the security or confidentiality of such transmissions.

Sharing of Information

Akron Physician Wellness Initiative (APWI) does not sell, trade, or rent personal information about its site visitors, or any personal information collected from its site visitors.

In the course of doing business, we employ a small number of other companies to perform a limited number of functions on our behalf. For example, we may provide information as necessary to companies that host our online platforms or send communications on our  behalf. These service providers may have access to information about you if it is needed to perform their functions for us, but they are not authorized by us to use or disclose such information except as necessary to perform services on our behalf or comply with legal requirements, and they are required to maintain the information in confidence.

Our organization is hosted on the Wix.com platform. Wix.com provides us with the online platform that allows us to explain our services to you. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall.  

 

Data Retention

We will retain your information for only as long as your account is active or as needed to provide you services, comply with our legal obligations, resolve disputes, or enforce our agreements.
